Description

Hospice Registered Nurse (RN) Weekend Warrior Manageable caseloads, flexibility, and meaningful hospice work that allows you to focus on quality patient care.

Join our team as a Weekend Warrior RN, supporting patient care from Friday through Monday. This full-time position offers flexibility in scheduling. For example, a typical schedule may start around 2:30 PM on Friday and continue through Monday morning (around 10:00 AM), but we are open to adjusting hours to fit the right candidate while maintaining weekend coverage.

Schedule, Hours & Territory • Weekend coverage from Friday through Monday with flexible start and end times to fit your schedule. • This role includes on-call responsibilities, along with completing scheduled visits as needed. • Crown Point, IN, and the surrounding communities within approximately a one-hour driving radius.

Key Responsibilities • Conduct comprehensive patient assessments and develop individualized plans of care • Provide skilled nursing care in accordance with hospice standards and physician orders • Coordinate care with physicians, caregivers, and interdisciplinary team members • Educate patients and families on symptom management, disease progression, and end-of-life care • Document assessments, interventions, and care plans accurately and in a timely manner in the EMR HCHB (Homecare Homebase) • Participate in interdisciplinary team meetings and care planning • Support after-hours patient needs as part of an on-call rotation when applicable

Qualifications

• Active Registered Nurse (RN) license • Hospice or home health experience preferred • Strong clinical assessment and care coordination skills • Ability to work independently in the field • Strong communication and documentation skills • Experience using HCHB (Homecare Homebase) preferred

Field Role Requirements • Active driver's license • Ability to pass a criminal background check • Ability to pass a Motor Vehicle Record (MVR) check • Current TB testing in accordance with agency policy • Current CPR certification as required for the role
