Physical Therapist (PT) – Home Health

Guardian Angel Home Care

Pay: $80,000 – $160,000 per year
Schedule: Full-Time | Part-Time | Per Diem Opportunities Available

We are currently seeking Physical Therapists (PTs) to provide home health services in the communities we serve.

New graduates and experienced PTs are encouraged to apply.


About the Role

As a Home Health Physical Therapist, you will provide individualized rehabilitation care directly in patients’ homes. You will evaluate patients, develop treatment plans, and guide them through recovery while helping them regain independence and prevent hospital readmissions.

Home health allows physical therapists to:

• Work one-on-one with patients
• Build meaningful patient relationships
• Create flexible schedules
• Focus on functional recovery in real-world environments


Responsibilities

• Perform comprehensive patient evaluations in the home
• Develop individualized treatment plans based on physician orders
• Provide therapeutic exercises and mobility training
• Educate patients and caregivers on home exercise programs
• Implement fall prevention and safety strategies
• Document patient progress in EMR systems
• Communicate with physicians, nurses, and interdisciplinary care teams
• Adjust treatment plans based on patient progress


Minimum Qualifications

• Active Physical Therapist (PT) License in the state of practice
• Graduate of an accredited Physical Therapy program (DPT or equivalent)
• CPR / BLS Certification
• Valid driver's license and reliable transportation


Preferred Qualifications

• Home Health experience
• Familiarity with EMR documentation systems
• Strong communication and patient education skills
• Ability to work independently in the field
