Bon Secours Maryview Medical Center is a 25-bed acute rehabilitation unit dedicated to helping patients who are recovering from various injuries or illnesses. In partnership with Lifepoint Rehabilitation, this facility provides 24 hours of nursing care and intensive inpatient therapy to provide an outstanding experience and quality outcomes for all of the patients we serve. 

How you'll contribute 

A Case Manager who excels in this role:

  • Participate in pre-admission screening as requested

  • Conduct initial assessments

  • Develop individualized treatment plans

  • Provide age-appropriate therapeutic interventions

  • Evaluate patient and family response to treatment

  • Document the course of patient care including progress made and continuing need for treatment

  • Establishes and maintains a therapeutic relationship with patients and families

  • Provide patient and family education on tasks and resources that will assist with continued functional independence

  • Obtain pre-certification and re-certification of patient's insurance coverage as appropriate according to policy and procedure

  • Participate in continuing education/professional development activities

  • Communicate and demonstrate a professional image/attitude for patients, families, clients, coworkers and others

  • Contribute to client relations and marketing activities

  • Maintain competency,  licenses/ certifications,  and memberships in applicable professional organizations

  • Other duties as assigned

What we're looking for:

  • Current Registered Nurse or Social Work licensure or Healthcare professional licensure as Respiratory Therapist, Physical Therapist, Speech Language Pathologist or Occupational Therapist.
  • Certification in Case Management or Rehabilitation Nursing preferred; for example, Commission for Case Manager Certification (CCM); Association of Rehabilitation Nurses (ARN) certification, American Case Management Association (ACM) or Board Certification in CM by the ANCC e.g.: RN-BC
  • Minimum of 2 years social work or case management experience in an inpatient setting highly preferred; acute/rehabilitation hospital experience preferred.
  • Effective oral and written communication skills in English, additional languages preferred.
  • Basic computer skills in excel, word, outlook, power point, etc. required.
  • Must have good organizational skills, time management skills and analytical ability in order to interpret information and carry out duties independently
  • Must be cooperative and have the desire to be a team player.
  • Must recognize and observe confidential
