JOB DESCRIPTION AND POSITION REQUIREMENTS
The Penn State College of Medicine, Department of Psychiatry, in Hershey, PA is seeking a part time Care Coordinator. The temporary, part-time (up to 20 hours per week), Care Coordinator for the Perinatal TiPS Program at our Northeast Drive Clinic, plays a critical role in facilitating outreach, networking, and support services for the program. This position serves as the primary liaison between community providers, families, and internal teams, ensuring efficient coordination of services and effective communication. The Care Coordinator will manage program documentation, coordinate networking activities, and oversee the organization of various meetings and events.
Key Responsibilities:
  • Call Management and Documentation:
    • Field incoming provider calls, route to appropriate on-call physicians, and document calls in various databases (Beacon, ACCESS, Excel).
    • Call families to coordinate services, provide provider lists, and update PCP offices on patient statuses.
    • Fax or mail relevant patient and provider information as needed.
  • Outreach and Networking Coordination:
    • Plan and coordinate outreach activities, networking events, and maintain professional relationships with behavioral health providers, insurance companies, and community organizations.
    • Maintain up-to-date resources, including a provider resource list and newsletters, and represent TiPS at statewide and local meetings.
  • Program Operations:
    • Coordinate physician on-call schedules and manage notifications using scheduling tools and communication systems.
    • Train new staff, organize team onboarding materials, and ensure program resources are current.
    • Maintain accurate records of program expenditures and assist with the development of marketing and outreach materials.
    • Report development
  • Meeting Facilitation and Administrative Support:
    • Organize and facilitate monthly staff meetings, care coordinator subgroup meetings, and statewide TiPS program meetings.
    • Take meeting minutes, prepare agendas, and provide updates on outreach and program outcomes.
    • Review and analyze program data to support strategic outreach efforts.
  • Patient and Provider Support:
    • Schedule evaluations and therapy intakes, send rating scales to families, and ensure physicians have necessary documentation.
    • Coordinate interpreters and provide administrative support for patient-related needs.

Qualifications:
  • Strong organizational, communication, and interpersonal skills.
  • Proficiency in Microsoft Office Suite (Excel, Publisher, Access) and other software tools (Beacon database, Qgenda).
  • Ability to handle confidential information with professionalism.
  • Experience in behavioral health, healthcare coordination, or a related field is preferred.
