Overview
The Home Health Nurse is a licensed professional who administers skilled nursing care in the home to clients requiring professional nursing service. These services are performed in collaboration with the other members of the home health multidisciplinary team, in accordance with the physician's orders and under the direction and supervision of the Clinical Supervisor.
Responsibilities
  • Skilled Nursing Care and Comprehensive Assessment
  • Teaching, Supervision, and Counseling Clients and Caregivers
  • Interdisciplinary collaboration
  • Problem solving and critical thinking skills
  • Monitors patient progress to ensure accomplishment of patient goals
  • Oasis documentation
  • Provides appropriate care for a range of physician-ordered treatments (PleurX Drain, Finger stick Coagulation checks, wound care, wound vacs, IVs, phlebotomy, Trachs, Peg Tube, Ostomies, Telehealth)

Qualifications
Minimum of one (1) years recent experience as a professional nurse in a clinical setting. Home Health experience preferred. Must complete Point of Care Testing training as part of Department Orientation.
