SUMMARY: Assist individual to develop self-directed budget and with ongoing management of self-directed budget and services. Manage and direct the budget pursuant to approved plan.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.
  • Promote person centered planning by working with individual and circle of support to develop a self-directed budget and staff action plan.
  • Maintain and update self-directed budget and staff action plan based on individual's needs and wants.
  • Provide support and training to individuals and their circle of support regarding ongoing decisions, responsibilities, and tasks associated with self direction.
  • Facilitate two face to face planning meetings a year to review and update the self-directed budget and staff action plans.
  • Properly document services provided and maintain all plans and records according to Medicaid and NYS standards.
  • Monitor self-direction expenditures to ensure that spending does not exceed self-directed budget.
  • Other duties may be assigned.

Requirements
EDUCATION and/or EXPERIENCE: Bachelor's degree in health/human service field or RN preferred; and one year experience working with people with a developmental disability. May consider an Associate's degree in a health/human services field with at least two years experience working with people with a developmental disability.
