Physical Requirements:

  • Ability to stand/walk for up to 100% of the work day.
  • Ability to assist in bathing and dressing of residents with disabilities.
  • Must be able to access and navigate full campus.
  • Ability to independently push a wheelchair with a resident weighing up to 200 pounds.
  • Ability to independently lift, balance, and carry up to 30 pounds.

General Summary:

The PT Overnight Shift Certified Medication Aideย is responsible for providing personal and non-personal care assistance to residents when there is a need and following each resident's personal plan of care; assists residents so that current living conditions can be maintained for as long as is safe and appropriate; promotes independence with daily activities while giving assistance to each resident.

In addition to the above-listed responsibilities, the Certified Medication Aide administers medications in an accurate and timely manner and may function as a Team Leader for the shift as assigned.

Essential Job Duties:

  • Monitors the work of the Assisted Living personnel assigned to her/his shift, if serving as Team Leader.
  • Monitors/performs direct care of residents as outlined in each resident's plan of care.
  • Administers direct care to residents as necessary.
  • Makes daily rounds to residents.
  • Gives and receives accurate shift reports.
  • Records accurate and timely resident progress notes, condition change notes, and other documentation as required and/or assigned.
  • Keeps physicians and families informed of each resident's condition, as required.
  • Administers medications (includes preparation, administration, and charting) and compliance with goal of 100% accuracy.ย  ย  ย  ย 
  • Completes all assigned tasks and items listed on shift checklist.
  • Keeps Charge Nurse, Nursing Coordinator, and Assisted Living Director informed of the status of each resident.
  • Assists with meal service including table set-up, serving, tableside assistance, apartment deliveries, beverage service, dining room cleaning, etc.
  • Records accurate and timely resident progress notes, condition change notes, and other documentation as required and/or assigned.
