1.0OVERVIEW

Area Clinical Manager role is responsible for planning and coordinating a comprehensive program to improve clinical outcomes and enhance rehabilitation value to clients while growing services throughout the community continuum of care. The Area Clinical Manager is part of TransPerfect Health's leadership team supporting DORs within assigned area sites and supervises all members of the facility rehabilitation team where an acting DOR.

2.0DESCRIPTION
  • Provide clinical and educational support to ensure superior clinical outcomes and community utilization
  • Assist in recruitment, interviewing and selection of therapy staff
  • Support, training and orientation Director of Rehab/Therapy staff
  • Completion, Interpretation, Education and Trend Analysis of facility chart audits within territory
  • Responsible for utilization growth and development of rehabilitation department within assigned communities
  • Assist in maintenance of clinical quality standards
  • Assist Director of Rehabilitation in the identification and implementation of clinical and community program development needs
  • Assist Director of Rehabilitation and Operation in promoting a positive work environment between employees, client, facility
  • Deliver Rehabilitation Services: Evaluation and Treatment as indicated based upon licensed scope of practice; maintaining a 25% productivity
  • Function as Director of Rehabilitation in primary assigned community as indicated
  • Mentor to rehab team members
  • Serve as an additional layer of liaison support between Rehab Department/TransPerfect Health & community
  • Complete all other tasks that are deemed appropriate for this role and assigned by the manager/supervisor
3.0REQUIRED SKILLS
  • Knowledge and Proficiency of therapy software, Net Health, and community bases EMR (i.e., PCC, MatrixCare)
  • Detailed knowledge of Patient Driven Payment Model (PDPM)
  • Ability to read and interpret clinical and performance management reporting
  • Ability to travel to assigned community sites
  • Ability to maintain professionalism in all situations
4.0REQUIRED EXPERIENCE AND QUALIFICATIONS
  • Registered PT/OT/ST with current state license
  • Knowledge of Assigned State Healthcare and CMS regulations
  • Telehealth
  • 5+ Years of leadership role within the Geriatric Rehabilitation Industry
5.0DESIRED SKILLS AND EXPERIENCE
  • Clinical and Operation experience within the Senior Living and Geriatric Space
  • Supervisory role within the therapy healthcare space
