📍 Location: Rutland, Vermont
📅 Contract: 13 Weeks
🕒 Schedule: Monday–Friday, 8:00 AM–4:00 PM
⏱️ Guaranteed Hours: 37 hrs/week
Position Overview
Seeking an experienced Home Health RN to provide comprehensive nursing care to patients in the home setting. The RN will serve as a case manager, develop and coordinate individualized plans of care, complete OASIS documentation, and collaborate with patients, families, providers, and interdisciplinary teams.
Requirements
Minimum 3 years of experience of Home Health RN
OASIS experience required
Vermont RN license required upon acceptance
Experience with home health nursing and case management
Must provide Hospice coverage while on call
Strong documentation and care coordination skills
Ability to perform patient/caregiver education and manage complex patient needs
Must be comfortable with high-tech skills including:
IV use and maintenance
Wound VAC
Complex wound care
Ability to work independently and collaboratively with LPNs and interdisciplinary teams
Must participate in on-call and weekend rotation
Holiday coverage required
Key Responsibilities
Assess patient and caregiver needs using the nursing process
Develop, implement, and evaluate individualized plans of care
Serve as case manager for assigned patients
Complete and maintain accurate OASIS documentation
Provide skilled nursing interventions and patient/caregiver education
Coordinate care with physicians, providers, therapists, social workers, vendors, and other team members
Manage wound care, IV therapy, wound VACs, and other high-tech nursing needs
Coordinate admissions, transfers, and safe discharges
Identify and coordinate interdisciplinary services
Participate in case conferences and peer review audits
Maintain timely and accurate documentation
