Job description

Randolph, VT (Only QUALIFIED Healthcare Professionals accepted) Home Care-RN - Home Care Case Manages maintain a case load of home care clients through assessment, plan of care, coordination and implementation, of evidence based care for each client, with the support of an interdisciplinary care team. This includes but is not limited to: appropriate treatment intervention, evaluation of outcomes, and utilization of services to ensure that patient care needs are met. Will participate in on-call and weekend rotation.
Community Attractions
Service area to include: Randolph, Brookfield, Day shift - 8:00am - 4:30pm, Monday - Friday w/rotating weekends. Sharon, White River Junction, Woodstock, Norwich, VT areas and Hanover, NH area** On-Call - Home Health nurses will be on-call from 4:30 pm - 7:30 pm to ensure the late discharges are processed and urgent patient needs are met.
