Registered Nurse (RN) – Hospice Case Manager

Location: Lehigh Valley, PA (ZIP Code 19518 and surrounding areas)
Schedule: Full-Time | No Nights, No Weekends

Position Summary

We are seeking a compassionate and experienced Registered Nurse (RN) Hospice Case Manager to join our interdisciplinary hospice team serving the Lehigh Valley area. This role focuses on providing high-quality, patient-centered care to individuals and their families during end-of-life, while supporting comfort, dignity, and quality of life.
