DaVita Integrated Kidney Care (IKC) is seeking a dedicated and knowledgeable RN or LCSW with strong clinical acumen to step into a pivotal role as a Palliative Care Clinician. This is an opportunity to play a vital part in specializing in and growing a national kidney care palliative program.

You will serve as a subject matter expert and the central point of contact between our palliative care team and broader kidney care teams. As a member of our team, you will be accountable for triaging complex kidney patients and ensuring a seamless transition to the appropriate palliative resources.

DaVita Integrated Kidney Care (DaVita IKC) is the integrated care division of DaVita Inc. working on DaVita's vision to provide integrated care to all ESRD patients, who are some of the most medically complex and vulnerable patient populations in the US. What You'll Do

As the Palliative Care Clinician, you will use your clinical expertise to help guide kidney patients and teammates. Your primary responsibilities will include:

Expert Clinical Review and Triage:

Serve as the point of contact for incoming patient referrals to the Kidney Palliative Care Program.

Using robust clinical judgment and insight, conduct medical record reviews and collaborate with the care team to determine the advance care planning needs of chronic kidney disease (CKD) and End-Stage Kidney Disease (ESKD) patients.

Seamless Care Coordination and Communication:

Provide clear, actionable guidance and next steps for the palliative care RN or Social Worker to ensure a warm and efficient handoff.

Provide clear and consistent communication across the interdisciplinary team regarding patient enrollment and the plan of care.

Consultative Partner to Care Teams:

Act as a dedicated consultant to provide timely education and guidance to peers.

Identify trends, highlight educational opportunities, and act as a collaborative partner with leadership on quality improvement initiatives.

What We're Looking For

Registered Nurse (RN) or Licensed Clinical Social Worker (LCSW) with an active, unencumbered license.

Graduate from an accredited school of nursing or social work required.

Minimum of ten (10) years' experience in clinical nursing or social work required.

Hospice or palliative care experience required.

Renal nursing or social work experience preferred.

Proven expertise in medical record interpretation, patient assessment, and care planning.

Exceptional communication skills, with the ability to provide clear, consultative feedback to clinical peers.

A strategic thinker who can identify trends, solve problems, and work collaboratively to improve processes.

Work from home office with demonstrated ability to work independently and manage time effectively in a dynamic, remote environment.

Basic computer skills and proficiency in MS Word and Outlook required.

Home office with internet connectivity at a minimum of 1MB upload and 1MB download speed required.

Ability to work on a Monday- Friday schedule. We serve patients nationwide.
