Responsibilities:

  • Assessing psychosocial needs, adjustment to illness and community re-entry barriers by completing comprehensive admission intake/psychosocial assessments, trauma informed assessments and comprehensive care plans per policy.
  • Provide counseling- individual, group and family.
  • Facilitate comprehensive care plans meeting with interdisciplinary teams, resident neighbors, families / designated representative.
  • Explore discharge planning/community options, including TBI waiver, NHTD waiver, OPWDD waivers and traditional discharge services.
  • Maintain communication with Open Door, Front Door programs when assisting with community re-entry
  • Interface with in house OASIS program, connections with the justice system.
  • Complete entitlements/benefits.
  • Maintain open lines of communication with designated representatives/legal guardians.
  • Communicate with all disciplines in building to build strong alliances and work groups.
  • Participate in Performance Improvement activities.
  • Enhance professional growth through participation in educational in services and community involvement.
  • Master Level Supervision available, hours can be flexed, availability to work weekends.

Requirements:

  • Bachelor of Social Work (BSW) degree, or Bachelor degree in a related field with one year experience in social work or case management.
  • Dresses professionally and communicates in a clinical and professional manner.
  • Experience in office technologies, computers, word processing and software programs. Has good organizational skills. The ability to communicate well with department staff. Has a pleasant and professional telephone persona.
  • Must be able to read, write, speak and understand the English language.
  • Possess the ability to make independent decisions when circumstances warrant.
  • Must have patience, tact, cheerful disposition and enthusiasm, as well as be willing to handle personnel based on whatever maturity level at which they are currently functioning.
  • Must possess the leadership ability and the willingness to work harmoniously with and supervise other personnel.
  • Ability to plan, organize, develop, implement, and interpret the programs, goals, objectives, policies and procedures of the facility.
  • Must maintain the care and use of administrative supplies, equipment, etc., and maintain the work area in a clean, safe and attractive manner.
  • Must be knowledgeable of computers, system applications, and other office equipment.

  • Rate will be commensurate with experience.
