What You'll Do
  • Perform daily skin assessments and administer prescribed treatments
  • Provide wound care and preventive skin integrity interventions
  • Monitor wounds and resident progress while communicating updates to physicians and families
  • Accurately document treatments, observations, and progress notes
  • Collaborate with the Director of Nursing and interdisciplinary team on treatment plans
  • Support infection prevention and skin integrity initiatives
  • Educate staff on wound prevention, pressure injury prevention, and treatment protocols
  • Ensure compliance with all clinical documentation and regulatory standards
What We're Looking For
  • Current Texas LVN or RN license in good standing
  • Skilled Nursing Facility (SNF) experience required
  • Strong clinical assessment and documentation skills
  • Compassionate, dependable, and detail-oriented approach
  • Wound care experience preferred (certification is a plus but not required)
