Key Responsibilities:- Conduct comprehensive assessments of neurological and spinal cord injury patients.
- Develop and implement individualized care plans based on clinical findings.
- Manage and respond to neuro-related complications such as autonomic dysreflexia and spasticity.
- Educate Veterans and caregivers on condition management and rehabilitation techniques.
- Collaborate with interdisciplinary teams to coordinate holistic, Veteran-centered care.
- Document all nursing care and assessments accurately in VA electronic health records (CPRS).
- Participate in patient safety initiatives and quality improvement programs within the VA system.

- Unlike ICU where itโs self-scheduling, the CLC will create the schedule for the LTC RNs 30 days in advance
- 12-hour shifts
- Rotating Weekends and Holidays

Job Description:
Long-Term Care (LTC) Registered Nurses (RNs) at a Veterans Affairs Medical Center (VAMC) provide compassionate, comprehensive nursing care to veterans with chronic illnesses, disabilities, or those in need of end-of-life care. These RNs typically work in Community Living Centers (CLCs), which function like skilled nursing or nursing home units within the VA system.
Key Responsibilities:ยทย ย ย ย ย Deliver direct patient care including assessments, medication administration, wound care, and monitoring of vital signs.
ยทย ย ย ย ย Develop and implement individualized care plans in collaboration with interdisciplinary teams.
ยทย ย ย ย ย Support patients with activities of daily living (ADLs), such as bathing, dressing, feeding, and mobility assistance.
ยทย ย ย ย ย Monitor changes in residentsโ physical and mental health and promptly report concerns to the medical team.
ยทย ย ย ย ย Provide emotional support to veterans and their families, particularly in palliative and hospice care settings.
ยทย ย ย ย ย Maintain accurate documentation in accordance with VA policies and electronic health record standards.
ยทย ย ย ย ย Participate in quality improvement initiatives, infection control, and safety protocols.

RN Requirements:
- Must have BSN
- Must have at least one year of recent experience in LTC/SNF
- AHA BLS, VA will not accept non-AHA certifications.

Qualifications- Currently licensed Registered Nurse (RN) in state of employmentย
- Previous experience as a Registered Nurse is preferred
- Excellent interpersonal and client care skills
- Valid driverโs licenseย
- First aid/CPR certification requiredย
