Hours: Monday - Friday, Days 8am to 430pm, 2 Holidays a year

Location: Community, Skilled Nursing Facility, and Hospital Visits

Position Summary: The Hospice Float RN will provide flexible, compassionate care across the hospice program, supporting patients and families during their end-of-life journey. Evaluate and admit patients and families for Hospice services. Coordinate care and services in collaboration with the interdisciplinary team. Provide education to patients, families, and providers regarding hospice philosophy. Provide an ongoing assessment of patients, ensuring appropriate initiation of care plans, services/levels, and assessment information that accurately reflects the patient’s clinical, functional, and psychosocial needs. Serves as a resource for patients and families through the end-of-life process; ensuring quality of life, best practice hospice care, and patient/family’s wishes are maintained and met with dignity and respect

Schedule: Monday - Friday, Days, 2 Holidays a year

Sign On Bonus: up to $15,000

Minimum Qualifications:

  • New York State licensed Registered Nurse
  • ASN required
  • One year of RN experience as a clinical nurse in a hospital or home care agency

OR

  • BSN and a minimum of six months experience in hospital or home care.
  • Hospice experience preferred.
  • E-Record (electronic medical record systems)
  • Current NYS Driver’s License

Physical Requirements: The position involves frequent sitting, standing, and walking. Occasional bending and pushing may be required. Tasks such as kneeling, squatting, reaching, twisting, balancing, climbing, and pulling are required infrequently.

Salary Range: $73,652 - $103,854

The referenced pay range represents the minimum and maximum compensation for this job family. Individual annual salaries/hourly rates will be set within the job’s compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations.
