MJHS is a large not-for-profit health system in the Greater New York area. Our range of health services include home care, hospice and palliative care for adults and children, rehabilitation and nursing care at Menorah and Isabella Centers, and the research based MJHS Institute for Innovation and Palliative Care. We also offer Elderplan/HomeFirst: health plans for Medicare and dual-eligible individuals. As a not-for-profit organization, many of our programs and services are made possible through the generosity of grateful families, corporate donors and grants, as well as our own employees.

Our MJHS Medical Associates, P.C. is a group of Nurse Practitioners, Physician Assistants, RN Case Managers and LPN's who provide care to Elderplan members who are residents of assisted living and long term care facilities, as well as to those living at home.

In collaboration with physician, provides and coordinates primary care services to an assigned panel of Advantage members who reside in participating long term care facilities. Insures development of care planning and access to preventive care services in accordance with member and Plan health outcome goals and objectives. Acts as clinical resource to the participating providers and facilities.

  • Provide and/or coordinate primary clinical care services for assigned Nursing Home Residents
  • Acts as team member and clinical resource to participating providers/facilities
  • Acts as resource and contact for member/designated representative regarding all aspects of Advantage member care plan
  • Masterโ€™s degree in nursing, graduate of accredited Nurse Practitioner program required
  • One to two years clinical experience in role of nurse practitioner required
  • Minimum one year in geriatric specialty
  • Previous clinical RN experience in acute, subacute, and/or long-term care setting preferred
  • Prior experience in manage care a plus
  • NYS RN/NP License, Registration, DEA certificates
