Job description

GuildCare, an innovative community-based Adult Day Health Care Program, seeks a full time NYS Licensed Registered Nurse to work with our multidisciplinary team in its Manhattan location.

We provide services to adults with chronic medical conditions and vision impairments. 

Responsibilities include nursing assessments, treatment planning/implementation and health care coordination and individual and group health education.

Qualifications:

Graduation from an accredited school of nursing.  Current license from State Board of Nursing.  Experience as a professional nurse, preferably in the geriatric field. Requires 3-5 years recent staff experience in Med/Surg., Long Term Care or Community Health and excellent communication and documentation skills. Excellent assessment and computer proficiency in MS Word. UAS-NY certified and proficiency preferred.  

Responsibilities:

  1. Completes a nursing assessment for each registrant based upon history, interviews, with registrant and family/collateral sources, and reports from private physician.
  2. Develops, implements, monitors and evaluates nursing treatment plans for all registrants. Re-evaluates nursing needs periodically and revises treatment plans as indicated.
  3. Monitors all medications taken by registrants while in attendance.
  4. Insures established inventory standards for medicines, solutions, supplies and equipment.
  5. Is responsible for appropriate handling of medical emergencies, including educating other staff members in appropriate tasks and procedures.
  6. Maintains a safe, clean environment; maintains medical equipment and supplies in optimal condition.
  7. Serves as consultant to other staff in matters of safety and infection control.
  8. May supervise nursing students and/or volunteers.
  9. Provides individual health counseling to registrants and family as needed
  10. Leads scheduled health education and counseling groups.
  11. Counsels other staff regarding any special handling or techniques required by any registrant.
  12. Acts as liaison between the GuildCare program and its Medical Director and the registrant’s private physicians.
  13. Participates in Performance Improvement and Quality Assurance activities.
  14. Completes all required records and reports on a timely basis.
  15. Attends all required staff meetings and conferences.
  16. Along with all other GuildCare staff, performs general assistance tasks on a regular basis. These include: escorting registrants throughout the space, including to and from the restroom, assisting with transportation, serving meals, cleaning up from meals as needed, and filling in for absent coworkers.
  17. Works with other team members to foster the interdisciplinary team approach.
  18. Provides all required nursing care and personal care as needed.
  19. Observes and implements the registrant’s Bill of Rights.
  20. Fills in as the acting Program Director in the Program Director’s absence.
  21. Responsible for maintaining confidentiality of registrant protected health information.
  22. Responsible for following the LGI Workforce security policy; will access information systems and ePHI authorized by Clinical Authorization only.

    23. Physical demands:

    • Standing for long periods, walking, lifting/transferring registrants, using lifting equipment when required.
    • Bending, stooping, squatting, twisting, reaching above and below shoulder level.
    • Able to push/pull registrants in wheelchairs.
    • Able to lift and carry equipment and/or supplies.
