Job description

Summary:

Under supervision of the I/DD RN provides nursing and medical related services to individuals, and consultation to staff regarding health care related matters, and other Individual Program Plan (IPP) components for which responsible; functioning as an Interdisciplinary Treatment Team (ITT) member, and assuring compliance with all applicable standards.

Essential Job Functions:

  • Assure and carry out all medical treatments prescribed by physician/dentist
  • Administer and monitor the security of medications to individuals per established policy and procedure
  • Assure and monitor health care services in accordance with Agency and program policy and procedure. Serve as information resource to staff, parents, and programs
  • Handle medical emergencies in accordance with established protocols; accompany individuals to Emergency Department or medical appointments, if needed, and complete documentation of injury/visit in individual’s chart
  • Follow through immediately on medication changes and newly written orders, calling MD to clarify if necessary
  • Assure physician documents on medically related progress notes as necessary
  • Ensure orders on pharmacy prescriptions from designated pharmacy store, monitor prescription renewals, and assure medications are in supply at all times
  • Observe each individual for health status and hygiene and take appropriate action if called for
  • Monitor and submit requests for medical/nursing equipment; maintain medication storage area and medical supply cabinet
  • Alert all staff and I/DD RN regarding pertinent medical information via designated communication system.
  • Ensure treatments are completed and recorded with full date and time in the individual’s chart
  • Compare doctor’s orders to the individual’s Medication and Treatment sheets, note any changes, and notify the appropriate staff of any discrepancies. Secure physician review and sign off orders quarterly
  • Fulfill all required documentation including but not limited to medical related progress notes, seizure charts, menstrual chart updates, formulary, immunization records, etc.; maintain medical files and records under supervision of I/DD RN.
  • Acts as a liaison with other community service providers
  • Participate in reviews and weekly clinical meetings as required
  • Perform all other necessary duties relevant to the position as requested by the supervisor

Knowledge, Skills, and Abilities

  • Ability to work independently and motivate others
  • Ability to communicate effectively, both orally and in writing

Education and Experience:

  • Must be licensed and currently registered as a Licensed Practical Nurse by the State of New York State Education Department
  • Specialized training or one year of professional experience in treating the developmentally disabled preferred

Physical Requirements/Working Conditions:

  • Ability to stand/walk frequently
  • Ability to reach above shoulder level
  • Ability to turn/twist upper body
  • Ability to lift at least 25 lbs.
