Portage County Health & Human Services has a full-time Community Services Program Assistant opening!
Pay range starts at $19.95 / hr
What are the primary job duties?
Provision of independent living skills, household organization skills, money management, and assistance with medication reminders for consumers with Serious and Persistent Mental Illness enrolled in the Comprehensive Community Services Program.
  • Teach specific skills such as money management, community accessibility, laundry, meal planning, employment related skills, maintaining personal hygiene, and cleaning.
  • Organize and monitor day-to-day schedule when necessary.
  • Provide direction for organizing and scheduling household duties and routines.
  • Provide resources to consumers and their families for obtaining necessary household equipment.
  • Assist consumers and their families in providing medication reminders to take medication as prescribed.
  • Provide transportation to consumers for attendance of medical appointments, appointments related to their service plan, or for activities designated on the consumer's service plan.

Engage in the coordination of care for consumers on assigned caseload.
  • Regular contact with contracted caregiving service to provide updates on consumers and assure smooth transition to evening, weekend, holiday coverage for consumers and their families.
  • Weekly to daily contact with client's assigned CCS service facilitator to obtain clinical updates regarding clientele.
  • Attend weekly staffing for clinical consultation with CSP/CCS/Mobile Crisis/Medical Director

What are the minimum qualifications?
  • High School Diploma or equivalent required. Post High School classes or degree in Human Services field preferred.
  • Experience or training with the serious and persistent mentally ill population required.
  • Experience or training with in-home caregiving preferred.
  • Must be a licensed driver and have access to an insured vehicle.
