Job description

The Case Manager, Mental Health Professional (MHP) provides masterโ€™s-levelย clinical services within CHMHAโ€™s licensed residential programs to meet Washington Stateย residential staffing requirements for clinical oversight and direct therapeutic services. Thisย position delivers direct mental health treatment to residents, supports treatment planning andย documentation, and contributes clinical expertise to the on-site care team. This role is not aย supervisory or leadership position, but functions as a clinical resource to support non-MHPย case managers and residential staff by providing consultation, guidance on clinical interventions,ย and continuity of care within a multidisciplinary team model. The MHP ensures residents receiveย appropriate therapeutic support while supporting compliance with licensing, contract, and qualityย standards.

SCHEDULE

Monday-Friday, 8:00am-4:00pm (Full-Time, 40 hours/week)

RESPONSILBITIES

โ€ข Provide direct individual mental health counseling and therapeutic interventions toย residents in accordance with service plans and clinical need.
โ€ข Provide clinical consultation and guidance to residential staff and nonMHP case managersย through collaboration and shared clinical planning, as needed to support resident care; thisย role does not include formal supervisory authority.
โ€ข Contribute to meeting King County residential licensing requirements by providingย required clinical services and therapeutic support to residents, consistent with Mentalย Health Professional (MHP) standards.
โ€ข Serve as Primary Case Manager for assigned caseload, including advocating for andย coordinating resident needs and services; ensuring Service Plans, Crisis Plans, Continuingย Stay Reviews, and other documentation are completed on time and kept up-to-date; andย providing consistent contacts and skills training to work towards goals.
โ€ข Support the clinical work of nonMHP case managers through collaboration, sharedย documentation standards, and consultation on treatment planning, while maintaining aย nonsupervisory role.
โ€ข Coordinate with relevant internal and external departments to coordinate residentย referrals, ensure timely intake into program, and review of individualized plans.
โ€ข Collaborate with care team to ensure all residents access needed treatment, support, caseย management, and appointments, including transportation to external providers.
โ€ข Ensure all interactions represent the five principles of trauma-informed care, includingย safety, choice, trustworthiness, collaboration, and empowerment.
โ€ข Facilitate, monitor, and support medications and medication windows to ensure the sevenย rights of medication use are followed: right person, right medication, right time, right dose,ย right route, right reason (including refusal), and right documentation according to agencyย provided medication training and within assigned role responsibilities.
โ€ข Actively participate in Shift Change meetings and complete associated documentation.
โ€ข Complete daily documentation in electronic medical record on resident interactions,ย medications dispensed when assigned, and other required documentation, ensuring anย interaction or observation and subsequent documentation are completed for all residentsย across the team.
โ€ข Practice good financial stewardship of agency and contract resources, including how timeย is being spent, supplies are used, property is being maintained, etc.
โ€ข Meet identified key performance indicators and metrics for the position as identified forย the program, division, and agency.
โ€ข Participate in discharge planning and written summaries of treatment as needed for clientย referral and releases of information.
โ€ข Assist with and support on-site cleaning, including skills training with residents toย maintain personal and community spaces, laundry, and preparation for regular pestย treatments.
โ€ข Provide immediate crisis intervention and resolution as needed.
โ€ข Always maintain resident and staff confidentiality.
ย 

The Case Manager, MHP โ€“ Licensed Residential works with on-site care team to provide high standards of care within the licensed residential setting, including but not limited to:ย 
โ€ข Completing assigned work related to maintaining and adhering to licensingย responsibilities, such as Washington Department of Social and Health Services (DSHS) andย Department of Health (DOH), as assigned by supervisor.
โ€ข Providing skills training and assistance to residents in Activities of Daily Living (ADLs) andย Instrumental Activities of Daily Living (IADLs) per their service plan and individual needs,ย such as assistance in hygiene needs, feeding, personal hygiene, cleaning, finances,ย transportation, etc. Includes preparing resident meals when cook is not on duty and perย menu.
โ€ข Learning and executing skills and techniques promoted in the State of Washingtonย mandated training, Fundamentals of Caregiving.
โ€ข Coordinating care, such as scheduling, attending, and/or arranging transportation forย appointments and other case management needs.
โ€ข Maintaining, and documenting in, resident charts, activity logs, meal charts, etc. following agency, county, state, and federal guidelines.
โ€ข Facilitating regular group and individual activities, encounters, and specialized residentย events, such as outings and holiday gatherings.
โ€ข Assisting in admission and discharge planning per supervisor direction.
โ€ข Participating in fire drills, resident orientation, chores, and resident room maintenanceย during resident move-in and move-out.
โ€ข Monitoring and managing facility milieu for visitors, television time, and quiet times.ย 
โ€ข Attending all scheduled meetings and supervisions.
โ€ข Staying awake on all shifts.
โ€ข Performing other duties as assigned.
