Job description

Hospice Patient Care Manager

We are currently searching for a Patient Care Manager (PCM).

Under direction from the Director of Clinical Services, your primary focus will be to oversee & support the clinical team who is supporting individuals and families facing the challenges that arise from a diagnosis that is terminal and at the end of life. Job Responsibilities:
  • Supervise and provide direction to the clinical team.
  • Oversee Interdisciplinary Team (IDT).
  • Performs routine education on infection control, assessments, continuity of care, and compliance standards.
  • Coordinate field visits with the nursing team and as needed by the patient plan of care in an effort to support the patients and their families.
  • Understand the hospice philosophy, pain management, and the needs of the terminally ill and be able to convey that in a language the patient and family understand.
  • Demonstrate excellent interpersonal and communication skills.
  • Demonstrate Hope Health Care's agency values - Compassion, Dignity, Integrity, Professionalism, Collaboration, Balance, Growth and Ethical Behavior.
Qualifications:
  • Degree in Nursing - ADN, BSN, MSN.
  • Valid Texas RN License.
  • Valid BLS.
  • Minimum 2 years experience in the hospice setting.
  • Previous Patient Care Manager experience preferred.
  • Ability to pass background check and pre-employment drug screen.
