Job description

Title: Medical Records Clerk
Reports to: Medical Records Supervisor
FLSA Status: Non-Exempt
Personnel Supervised: None
POSITION SUMMARY:
The Medical Records Clerk scans into an electronic medical chart and follows HIPAA regulations, in an efficient and professional manner and in accordance with established standards.
MINIMAL QUALIFICATIONS:
Education: High School Diploma, or equivalent.
Experience: Demonstrates ability to communicate effectively orally and in writing. Demonstrates ability with computer skills.
Special Skills: History of working with an electronic health record preferred.
RESPONSIBILTIES AND PERFORMANCE EXPECTATIONS include, but are not limited to, the following:
  1. Be familiar with CFHC's program philosophies, goals and objectives. Be cognizant of and comply with all CFHC's policies and procedures, as well as state and federal regulations.
  2. Consistently coordinates work to achieve maximum productivity and efficiency.
  3. Maintains confidentiality based upon "need to know" according to CFHC Compliance Program and HIPPA (Includes all information, verbal, written, fax, phone, cellphone, e-mail).
  4. Will be held accountable for dissemination, copying or discussing of information not applicable to your duties/responsibilities and subject to disciplinary action including termination of employment.
  5. Support the goals and mission of CFHC, Inc.
  6. Knowledge/understanding of HIPAA and the importance of confidentiality.
  7. Maintain electronic medical records, according to policies and procedures of the community health center.
  8. Prepare paper medical record chart/electronic medical record chart for legal requests/subpoenas.
  9. Scan new patient information into electronic medical records at time of visits.
  10. Knowledge/understanding of record retention policies for paper charts.
  11. Daily work on behind the scenes mapping for combining information between systems.
  12. Maintain and scan documentations from basket in a timely manner.
  13. Ensure request for release of information are retrieved timely according to procedure.
  14. Contact Medical Records Supervisor with questions concerning release of information.
  15. Scan all medical records reports timely and according to procedure.
  16. Complete chart conversion and auditing.
  17. Improve continually with Computer Literacy.
  18. Perform other duties as assigned.

Equipment/Machinery Used in Position: Telephone (voice mail), computer, facsimile, copy machine, calculator, AV equipment (TV/VCR), overhead projector, laptop, cell phone, tablet
PHYSICAL REQUIREMENTS:
1. Requires 80% or more time spent sitting at a computer
2. Independently mobile
3. Ability to lift weight equivalents to what would be required when lifting supplies and equipment
4. Ability to adapt and function in varying environments of workload, worksites and work shifts
