Job description

Bluelake Homecare is hiring an Intake Coordinator to work with clients already in our pipeline and get them approved for care.

Once a client enters intake, you will assist in: gathering documents, completing Medicaid paperwork, and traveling to client homes when a family needs help getting forms done right. You will also be present at the in-home MDHHS assessment that determines whether a client qualifies and how many hours of care they receive each week.

What You Will Do

  • Move clients through intake to start of care

  • Collect and verify consents, physician paperwork, and Medicaid forms

  • Travel to homes to help families complete documents in person

  • Attend MDHHS in-home assessments and support the client and family

  • Prep families beforehand so they can clearly describe their daily needs

What You Need

  • Detail oriented and organized

  • Strong communicator, patient with seniors and families

  • Comfortable working independently in the field

  • Valid Michigan driver's license, reliable vehicle, current insurance

  • Able to pass a background check

Nice to Have

Experience with Michigan Medicaid, Home Help, MI Choice Waiver, home care, social services, or medical intake. Bilingual a plus.
