Description
The purpose of this position is to work directly with individuals and groups to provide evaluation, diagnosis, intervention, and treatment services for patients with substance misuse and substance-related health conditions, with a primary focus on opioid use disorder (OUD). This position serves as the MAT Coordinator, managing reporting requirements tied to funding deliverables, and supporting capacity-building efforts to serve unfunded and underinsured patients. The MAT Coordinator will operate in a community-based capacity, working within Nexus Care office locations (Beaufort and Bluffton) and other designated community sites to deliver OUD services, including coordinating treatment and medication services for uninsured or underinsured patients. This class works under close to general supervision according to set procedures, but determines how or when to complete tasks.
Examples of Duties
The essential functions listed below are those that represent the majority of the time spent working in this class. Management may assign additional functions related to the type of work of the class as necessary.
  • Conducts intake, screening, and assessment of biological, psychological, and social factors related to substance misuse and substance use disorders. Provides recommendations for evidence-based interventions and treatment.
  • Collaborates with patients, families, key stakeholders, and the clinical treatment team to develop individualized care plans.
  • Conducts individual, family, and group education, counseling, and psychotherapy sessions.
  • Provides crisis assessments and interventions, including emergency response, collaboration with community agencies, development of intervention plans, referral for psychiatric medication evaluation, and arrangement for appropriate care and services.
  • Provides case management to address identified needs for medical services, mental health care, housing, employment, childcare, social support, and other related services.
  • Prepares clinical documentation, reports, and correspondence, including intake packets, treatment plans, progress notes, treatment reviews, transition/discharge plans, court documents, mandated reporting, drug screening/testing, clinical charts, and related paperwork.
  • Oversees clinical and administrative functions related to MAT services, including conducting monthly reporting and tracking performance outcomes tied to funding deliverables.
  • Builds community capacity by coordinating and delivering opioid use disorder (OUD) services to unfunded and underinsured patients at Nexus Care and community-based locations.
  • Coordinates medication services and treatment plans for uninsured or underinsured patients identified with moderate to severe OUD.
  • Assists with the harm reduction efforts of the department related to OUD and overdose prevention.
  • Attends professional trainings for knowledge of best practices and to maintain necessary professional certification and licensure.
  • Participates in clinical supervision as required by regulatory agencies and department policy.
  • Assists families and clients with support through interpretive services as needed.
  • Performs related work as assigned.

Typical Qualifications
MINIMUM EDUCATION AND EXPERIENCE REQUIREMENTS:
  • Requires Bachelor's degree or education and training equivalent to four years of college education in business, liberal arts, or any field other than engineering or the hard sciences.
  • Over two years and up to and including four years of related experience or an equivalent combination of education, training, and experience.
  • Licensed Clinical Social Worker.
  • Licensed Addiction Counselor.
  • Licensed Professional Counselor.
  • Certified Addiction Counselor, preferred, or must be eligible within three years of employment.
  • Must possess and maintain a valid state driver's license with an acceptable driving history.

Supplemental Information
All applicants tentatively selected for a position with Beaufort County Council will be required to submit to urinalysis to screen for the illegal use of drugs and to a background check prior to appointment. Employment is contingent upon clearing both the drug screening and background investigation.
