The Care and Connectย Rural Health Transformation Project Hub (RHTP)ย Project Managerย contributesย to the Calling of Medical Teams International byย leading the planning, launch,ย scalingย and operational coordination ofย theย Care and Connect Hub serving Central Oregon. This role oversees project execution across people, processes, and partnerships to ensure the hub is implemented on time, within budget, and aligned with program goals to improve access to coordinated care and supportive services.ย The Project Managerย ensuresย responsible use of funds, strong partner engagement, and reliable implementation of workflows that support effective care coordination.ย

ESSENTIAL DUTIES AND RESPONSIBILITIESย 

Project Planning & Implementationย 

  • Leadย end to endย project management for the Central Oregon Care and Connect Hub, from startup through early operations.ย 
  • Develop andย maintainย detailed project plans, timelines, milestones, risk logs, and implementation checklists.ย 
  • Develop,ย implementย andย coordinateย aย phasedย hub rollout, ensuring readiness across staffing, workflows, systems, and community partners.ย 
  • Identifyย project risks and dependencies and implement mitigation strategies to keep deliverables on track.ย 

Budget & Resource Managementย 

  • Manage project activities within the approved Care and Connect budget, ensuring fiscal responsibility and transparency.ย 
  • Track project expenditures, resourceย utilization, and staffing allocationย tied to deliverables.ย 
  • Ensure project decisions align with budget constraints whileย maintainingย program quality and compliance.ย 
  • Proactivelyย facilitateย the growth of Medical Teams International programs by supporting the development of institutionally funded program proposals and concept notes,ย as well as supporting financial and program reporting to donors.ย ย 
  • Collaborate withย Financial Control and Performanceย Analystย andย US Programs (USP)ย Executive Directorย and Program Managersย to forecast needs,ย monitorย variances, support required reporting, share program information, and ensure data isย accurateย and upย toย date.ย 

Cross Functionalย & Community Coordinationย 

  • Serve as the primary project liaison among internal teams,ย community basedย organizations, health partners, and regional stakeholders.ย 
  • Coordinate onboarding, communication, and readiness activities with hub partners across Central Oregon.ย 
  • Facilitate regular project meetings, working groups, and implementationย check ins.ย 
  • Support alignment between hub operations and regional service delivery priorities.ย 

Operations & Workflow Developmentย 

  • Collaborate with Care and Connect leadership to design and implement standardized hub workflows, intake processes, communications and management procedures, and escalation protocols.ย 
  • Support integration of data tracking, reporting, and quality improvement processes.ย 
  • Ensure operational and program practices support equity, accessibility, andย trauma informedย service delivery.ย 

Monitoring, Reporting & Continuous Improvementย 

  • Develop and track project metrics tied to timelines, outputs, and early outcomes.ย 
  • Prepare clear, concise project status updates for leadership, funders, and partners.ย 
  • Identifyย opportunities for efficiency, scalability, and process improvement as the hub matures.ย 
  • Support documentationย requiredย for audits, compliance, and funding accountability.ย 
  • Ensure accountability is integrated into strategies, program design, monitoring and evaluations, recruitment, staff onboarding, training and performance management, partnership agreements, and highlighted in reporting.ย ย 

Other Duties:ย 

  • Ensure the Safeguarding of program participants and employees through promoting an environment of awareness and upholding the code of conduct provisions of Medical Teams Protection from Sexual Exploitation, Abuse & Harassment, Child & Vulnerable Adult protection, and harassment free workplace policies. Incumbents will remain alert and responsive to any child and adult safeguarding risks, acquire relevant knowledge and skills which will enable you to promote strong safeguarding practices, understand the child and adult safeguarding policy and procedures, and conduct yourself in a manner consistent with the Safeguarding code of conduct and behavior expectations.ย ย 
  • Utilize volunteers whenย appropriate.ย

EDUCATION, LICENSES & CERTIFICATIONS

Required:ย ย 

  • Bachelorโ€™s degree in public administration, health services, social services, business, orย a relatedย fieldย (or equivalentย experience).ย 

Preferred:ย ย 

  • Project management certification.ย 

EXPERIENCEย 

Required:ย ย 

  • 3 to 5 years of project management experience, preferably in healthcare, human services, care coordination, orย community basedย programs.ย 
  • Demonstrated experience managing projects with fixed or startup budgets.ย 
  • Experience working with diverse stakeholders, includingย community basedย organizations and public sector partners.ย 

Preferred:ย ย 

  • Familiarity withย grant fundedย or publicly funded program requirements.ย 
  • Experience supportingย hub based, coordinated care, or social service delivery models.ย 
  • Knowledge of Central Oregon communities, resources, and service systems.ย
