Job description

The Manager Hospital Care Coordination:
  • Provides overall management of the department team members.
  • Monitors length of stay and identifies way to reduce length of stay through case management and identification of safe alternatives to inpatient care.
  • Promotes and supports BayCare strategic directions and goals (including but not limited to reducing hospital readmission, observation length of stay, patient experience, and team member as customer).
  • Collaborates with ancillary departments, community agencies, and administration to achieve efficiency and effectiveness throughout the continuum of care.
  • Provides leadership for disease management, and care management processes, including identification of target populations, review of evidence-based guidelines, development of protocols/interventions and evaluation of outcomes.
  • Formalizes care coordination processes, roles, responsibilities, policy and procedures and continuously updates documents as improvements are implemented.
  • Oversees regulatory requirements.

Minium requirements
Education
  • Bachelor's degree in Nursing or Healthcare Management; OR
  • Master's degree in Social Work, Mental Health Counseling, Community Counseling, Human Services, or a related field

Experience
  • Minimum of 3 years of experience in Acute Care, Case Management, or Social Services
  • Minimum of 2 years of experience in a lead, coordinator, or manager role in a related field

Licensure / Credentials
  • Current one of the following:
    • Registered Nurse (RN)
    • Licensed Clinical Social Worker (LCSW)
    • Licensed Mental Health Counselor (LMHC)

Preferred Certifications
  • CCM (Certified Case Manager)
  • ACM (Accredited Case Manager)
  • CMAC (Case Management Administrator Certification)
  • CMGT-BC (Nursing Case Management Board Certified)
