Location: Phoenix, AZ

About Us

Dependable Health Services is opening a new hospice agency in Phoenix and is seeking a compassionate PRN Chaplain to provide spiritual and emotional support to hospice patients and their families.

Position Summary

The Hospice Chaplain provides spiritual care, emotional support, counseling, and bereavement assistance to patients, families, and caregivers of all faith backgrounds. This role is an important member of the interdisciplinary hospice team.

Responsibilities
  • Provide spiritual and emotional support to hospice patients and families
  • Conduct spiritual assessments and document care appropriately
  • Participate in interdisciplinary team meetings
  • Support patients of diverse cultural and religious backgrounds
  • Provide grief support and bereavement counseling as appropriate
  • Coordinate community clergy involvement when requested
  • Maintain timely and accurate documentation
