Position Overview

As a Health Coach at Synapse, you will work directly with clients to bridge the gap between clinical recommendations and real-world lifestyle changes. You will be the ultimate accountability partner, guide, and educator, helping clients navigate their unique health journeys spanning nutrition, stress management, sleep, and movement.

Key Responsibilities

  • One-on-One Coaching: Conduct personalized coaching sessions to help clients establish and achieve realistic wellness goals.

  • Care Coordination: Collaborate closely with our clinical team to align coaching strategies with patient treatment plans.

  • Behavioral Support: Utilize motivational interviewing and behavioral change theories to help clients overcome obstacles and build lasting habits.

  • Progress Tracking: Maintain detailed, accurate, and confidential client records tracking metrics, challenges, and breakthroughs.

  • Community Engagement: Potentially lead group coaching sessions, workshops, or contribute to educational wellness content.
