Job description

The Program Manager of Re-Entry, with the support of the Program Director, is responsible for overseeing and ensuring the successful implementation of all St. John’s Community Health re-entry program grants, contracts, and initiatives throughout the Inland Empire, with a particular focus on San Bernardino County. This position ensures that all program deliverables, reporting requirements, and compliance standards are met while working closely with staff, community partners, county agencies, and senior leadership to support justice-involved individuals returning to the community. The Program Manager oversees day-to-day program operations, including outreach, health and psychosocial education, care coordination, patient advocacy, housing and employment navigation, linkage to medical and behavioral health services, and other supportive services that address social determinants of health. The role is responsible for monitoring program performance, managing budgets in collaboration with the Finance Department, supporting program sustainability, developing strategic partnerships, and advancing innovative re-entry initiatives, including projects focused on pre-release engagement, workforce development, behavioral health access, homelessness prevention, and successful community reintegration for individuals impacted by the criminal legal system across the Inland Empire.

QUALIFICATIONS

Education & Experience

  • Master’s Degree preferred, or equivalent education and/or experience.
  • Five years’ related experience working for a social services or health organization in program development, evaluation, data analysis, program management, and grant writing including report writing.
  • Research experience in health services or social science research.
  • Working knowledge of health information systems and data needed to monitor/assess utilization, performance management, and health outcomes.
  • Experience preparing and delivering presentations, reports, and project/program briefs.
  • Extensive knowledge of development of evaluation tools, and evaluation of health programs, data analysis, and report writing.
  • Strong health care knowledge base required to strategically develop health program evaluations and manage health grants.
  • Have an expert understanding of the reentry process and its effect on those exiting incarceration; this includes individuals exiting local and county jails, juvenile detention centers, state and federal prisons, and other carceral systems.
  • Strong knowledge of community health evaluation tools, approaches, frameworks, and methodologies.

ESSENTIAL DUTIES AND RESPONSIBILITIES

Performs a combination, but not necessarily all, of the following duties:

  • Works with clinic manager, programs leadership, and other departments to support integration.
  • Review and audit staff charts and documentation related to projects under Re-entry and support programs according to organization standards.
  • Enhance and expand the program in ways that are aligned with organizational goals and that of the re-entry community.
  • Supervise staff and provide coaching/development support as needed.

Grant Management

This position will support the Program Director in:

  • Manage all existing and new grants and contracts by: tracking grants and contracts (ongoing monitoring of grant program goals and deliverables, monthly performance, budget, and expenditures); developing project/program work plans; developing internal reporting systems; writing reports; maintaining excellent historical records; and working with staff to ensure each project or program is meeting proposal conditions and expectations.
  • After grant award, review and address grant objectives and deliverables with program staff responsible for its implementation. Review and communicate award expiration dates, altered timeframes, milestones, and major changes in budgets to appropriate staff.
  • Work with program staff to develop and maintain all grant agreements and Memorandums of Understanding (MOUs) pertaining to grant awards.
  • Write reports for grants as assigned, including HRSA BPR report. Track, coordinate, write, and submit grant reports and contracts in a timely manner.
  • Communicate professionally with donors and partners to ensure effective grant management and advancement of program implementation.
  • Ensure patient data is entered in ECW and any external databases as it relates to grants and contracts.

Program Development and Management

  • Supervise program staff and provide development support to enrich individual productivity, work quality, and overall team engagement.
  • Provide management to re-entry programs at St. John’s.
  • Work collaboratively with other departments to identify and evaluate program opportunities according to overall strategic objectives.
  • With the Development Team, collaborate in the development of programs, including research, planning, implementation, and evaluation components.
  • Work with clinical team to integrate designated programs into clinical workflow.
  • Work collaboratively with the Finance Department & Director of Support Services to manage program budgets and ensure they are in good standing.
  • Track program outcomes as specified by grant objectives.
  • Ensure regular dissemination of activities or results of program planning and improvement efforts, including frameworks, program descriptions, articles, reports or decision tools, across departments and to external audiences, where appropriate.
  • Establish and maintain healthy relationships with external community-based organizations within the field of re-entry and abroad.
  • Seek and develop collaborative prospects to strengthen opportunities and resources available to the re-entry community and the organization as a whole.
  • Participate in external committees that broaden the organization.

Evaluation

  • Work collaboratively with the QI and Compliance Committee to measure, evaluate and improve program effectiveness including HRSA quality measures
  • Act as primary contact for outside evaluators on larger agency program evaluations.
  • Recruit and supervise interns, partners, or staff resources to execute evaluation projects;
  • Design and conduct internal evaluations as necessary for grant deliverables, including assisting Development Team with evaluation components for grant applications and quarterly/yearly reports.
  • Facilitate and analyze annual patient satisfaction surveys.
  • Develop mechanisms to monitor and measure program results to increase effectiveness, efficiency, and impact
  • Track program outcomes as specified by grant objectives
  • Attend meetings, trainings, and other work-related events as needed.
  • As an exempt position, evening and/or weekend hours may be required to ensure the fulfillment of all duties and responsibilities; and
  • Other duties may be assigned or may be modified as business needs dictate.

Supervisory Responsibilities

  • This position oversees all employees of assigned teams and is responsible for the performance management and hiring of the employees within the team.
  • Works closely with Clinic Manager and Medical providers.
