Job description

Job Summary:

The Nurse Practitioner/Physician Assistant is responsible for the delivery of quality medical care to patients of Midtown Health Care and functions as a member of the multi-disciplinary health care team within the scope of his or her medical practice.

Supervisory Responsibilities:

None.

Duties/Responsibilities:

  1. Provides direct patient care to clinic patients.
  1. Examines, diagnoses, and prescribes appropriate treatment plans for patients. Follow appropriate standards of care for each patient.
  2. Refers patients for appropriate specialty care services, lab and x-ray and any other ancillary services that are appropriate for patient’s management and care. Reviews all lab and x-ray reports for patients under his/her care on a timely basis and makes medical management decisions appropriately.
  3. Refers patients requiring hospitalization or assessment and management of conditions to supervising physicians or appropriate specialist. Follows patient’s progress with physician.
  4. Maintains health records for all patients and provides appropriate follow-up instructions through the use of electronic health records.
  5. Formulates, initiates, and monitors patient management plans for patients using protocols and/or consultation with a supervising physician.
  6. Provides continuity of care to patients and serves in rotation for after-hour care coverage.
  7. Ensures that patient and family are educated about diagnosis and the plan of care.
  8. Consults with physicians and other members of the health care team as necessary.
  9. Participates in specified health promotion, education, and/or prevention programs as needed.
  1. Participates in the review, development, and training of patient care policies, procedures, protocols, and standing orders.
  2. Provides teaching and clinical supervision to nursing staff and trainees.
  3. Accepts responsibility for remaining current with medical and health care trends and information that impact the medical practice.
  4. Assists in maintaining an efficient, safe, secure, orderly, and aseptic environment of care for all patients and employees.
  5. Performs other duties as assigned.

Duties 1 through 5 are designated as ADA Essential Functions and must be performed in this job. All other job duties are secondary functions.

  • Must be able to maintain strict confidentiality of sensitive information in accordance to HIPAA regulations.
  • Ability to comply with all occupational safety and health standards and all rules, regulations, and orders issued under the Occupational Safety and Health Act (OSHA).
  • Ability to utilize critical thinking and problem solving skills to address clinic’s needs.
  • Ability to interpret oral and written instructions and do appropriate follow-up.
  • Ability to work independently and as a team member.
  • Must have exceptional time management and organizational skills.
  • Ability to constructively participate in quality improvement efforts.
  • Ability to actively participate in marketing and outreach activities when requested and emergency response events when required.
  • Bilingual (English/Spanish) verbal and written skills highly desired.
  • Knowledge of advanced principles of health promotion, prevention, and motivation.
  • Knowledge and understanding of program requirements to maintain FQHC and PCMH status.
  • Ability to document, with clarity, all information relevant to a patient’s needs.
  • Ability to effectively present information in small group situations to patients, clinic staff, and the general public.
  • Ability to navigate EHR, Nexus, and communicable disease reporting systems.

Education and Experience, Licensure and Certification:

  • Graduation from an accredited school of nursing and a Nurse Practitioner/Physician Assistant program recognized by the Board of Nursing or from a program otherwise designated for Family Nurse Practitioners/Physician Assistant which has been approved by the State Board of Nursing.
  • Administrative experience as an APRN in implementing community health programs preferred.
  • Current unrestricted license to practice as an Advanced Practice Registered Nurse in the State of Nebraska is required or ability to obtain licensure.
  • Current Drug Enforcement Administration (DEA) license is required or ability to obtain licensure.
  • Must meet all credentialing requirements as required by Medicaid, Medicare, and insurances.
  • Current certificate in BLS required.
  • Must have a valid driver’s license and the ability to utilize own transportation for work purposes.
