Job description

Referral Manager, Lifesharing
Full Time, Monday through Friday 8:30A - 4:30P (with flexibility on occasional nights and weekends)
Hybrid in office / in field / remote position
Located in Blue Bell, PA
The Lifesharing Referral Manager is responsible for growing KenCrest's Lifesharing program through outreach and networking with internal and external stakeholders, marketing, and overall promotion of KenCrest and Lifesharing. It requires dynamic interpersonal skills, being adept and comfortable with networking, and growing the Lifesharing program.
Responsibilities include but are not limited to:
  • Being the point of contact for all incoming referrals both internal and external to KenCrest with a response time of 24 hours to all inquiries
  • Handle emergency referrals / respite care, placement, and billing coordination.
  • Responsible for all steps of intake starting with referral, assessment, matching, all the way through to placement.
  • Continuously build pool of available and vetted providers and respite providers.
  • Review completed and approved applications, schedule and complete home studies, coordinate visits, and work to finalize matches.
  • Conduct Introduction to Lifesharing Information Sessions.
  • Coordinate and gather all required documentation to meet licensing and KC Lifesharing standards and practices.
  • Be the face of KenCrest Lifesharing at service fairs, presentations, and conferences to promote KenCrest Lifesharing.
  • Update and maintain referral information for KPI and Performance Base Contracting.
  • Maintain and update referral and Provider information in systems as such as Evolv & Teams.

The ideal candidate for this role will have quality people and communication skills, be organized, successfully time manage, possess the ability to work independently and with a team, support positive approaches, and have experience with people with developmental disabilities and supporting family dynamics.
Qualifications
  • Bachelor's Degree in Human Services or related field
  • Minimum four (4) year's experience working with individuals with intellectual and development disabilities and Lifesharing
  • Experience with Residential referrals and intake processes, preferred
  • Valid driver's license
  • Clear criminal record and child abuse check in accordance with state requirements
