Summary:

The Meaningful Day Community Connection Coach supports individuals with intellectual disabilities in building meaningful, selfdirected lives within their communities. This role focuses on helping each person explore and develop their personal interests by creating opportunities for community inclusion, engagement, and growth. Community Connection Coaches assist individuals in forming natural relationships within their local communities, fostering connections that promote independence, belonging, and confidence that could ultimately lead to community employment.

The Community Connection Coach also collaborates with each individual to develop personalized routines and activity plans that reflect their goals, strengths, and interests. This includes supporting individuals as they participate in volunteer work, access community resources, and engage in experiences that enrich their daily lives. Through personcentered support and strong community partnerships, the Community Connection Coach empowers individuals to build a meaningful life connected to their community around them.

Key Relationships:

The key relationships for this role are the supported individual(s), Meaningful Day Coordinator, Community Integration Liaison, Outcome Specialists, KenCrest Residential Team and the Program Director.

Supervisory Responsibility: This position does not have supervisory responsibility.

Essential Functions of Position:

Leadership:

  • Integrating individuals into their local communities and facilitating activities that provide people with social capital and connections to their community.
  • Planning and structuring meaningful daily activities that align with the individual's likes and interests and providing support and transportation as needed in personal vehicle.
  • Assisting individuals with building relationships and maintaining those relationships by developing structure and routine into their programming.
  • Implementing actions and strategies as outlined in Individual Support Plans and Behavioral Support Plans to assist with goal attainment.
  • Teaching skills to develop, increase and maintain their independence.

Collaboration

  • Working with interdisciplinary team members, healthcare providers, family members and external support systems to ensure consistency and follow-through on support recommendations as deemed necessary.

Communication

  • Modeling and reinforcement of communication techniques to promote positive social interactions and relationship building between supported individuals and members of the broader community.
  • Facilitating effective communication by supporting individuals to express their needs and preferences within community settings, using their preferred communication methods.
  • Understanding communication needs and learning styles; adapting your communication to each individual, e.g. sounds, tone, body language.
  • Monitoring and reporting on progress toward goals and outcomes, including brainstorming solutions and strategies to address barriers to goal attainment.
  • Completing required support documentation including CPS Sheets, Behavior Support Charts, and Incident Management forms.

Problem-Solving and Follow-through

  • Observe, document, and support medical conditions.
  • Ensure the health, safety and wellbeing of the individual while navigating the community.
  • Act immediately in an emergency, including calling 911, administering CPR/FA, contacting supervisors or emergency on call as required.

Qualifications:

  • High School Diploma or GED.
  • Organizational and time management skills.
  • Good listening and communication skills.
  • Good writing skills to complete required documentation.
  • Ability to work independently in a community setting without direct supervision.
  • Able to adapt to changing environments.
  • Able to understand and use technology such as an iPad.

Physical Demands:

  • Regularly required to sit, stand and walk for periods of time
  • Occasionally climb and descend steps
  • Frequently talk and hear
  • Frequently required to use hands and fingers to handle materials and/or equipment
  • Occasionally required to drive or travel from location to location for periods of up to 1 hour
  • Be capable of visual and auditory scanning of the work environment to monitor the facility to ensure safe and effective operations.
  • Perform physical work, including the ability to exert or lift up to 30 pounds of force occasionally, and/or up to 20 pounds of force frequently, and/or up to 10 pounds of force constantly to maneuver objects.

Working Conditions:

This position requires use of your own vehicle to travel to individual(s) home for pick up and transporting the individual within the community for various activities and community outings. May be required to attend meetings in person, in the community or virtually. Occasional travel to satellite offices for in-person meetings or training.
