This position is based onsite as part of our home care service, allowing you to deliver therapy directly within patients' homes. As a Physical Therapist at Kaiser Physical Therapy, your primary responsibilities will include:

• Directing customized physical therapy treatments aimed at preventing disabilities and relieving pain.
• Overseeing the implementation of care plans by physical therapy assistants, ensuring compliance with established protocols.
• Conducting thorough evaluations of patients' functional capabilities using advanced diagnostic techniques.
• Assessing the home environment to recommend modifications that enhance functional independence.
• Administering therapeutic exercises, alongside instructional guidance on self-care and rehabilitation activities.
• Utilizing techniques such as massage and traction to support pain management and recovery.
• Evaluating the necessity for and fitting of prosthetic and orthotic devices, communicating adjustments to physicians as needed.
• Maintaining detailed clinical documentation, including initial assessments, care plans, and progress reports, compliant with OASIS criteria.
• Actively participating in interdisciplinary team meetings to collaboratively develop patient care strategies.
• Providing education to patients and families regarding therapy protocols and device use.
• Supervising Physical Therapy Assistants in accordance with state regulations.
• Staying accessible to support the Physical Therapy Assistant during patient care delivery.

Your role will involve working closely with a diverse patient population, including those recovering from surgery, managing chronic illnesses, or coping with age-related limitations. The hands-on nature of at-home therapy allows for tailored engagements that consider emotional and social factors alongside physical health. Your ability to connect with clients and foster trust will be fundamental in achieving positive rehabilitation outcomes.

The ideal candidate will have:

• A Doctorate in Physical Therapy (DPT) with current state licensure in good standing.
• A minimum of two years of professional experience, ideally in acute care or home care settings.
• Current CPR certification and a valid driver's license, including proof of automobile insurance.
• A strong commitment to quality care, ethics, and continuous professional development.
• Exceptional communication skills, with the ability to effectively interact with patients, families, and a multidisciplinary team.

While not mandatory, candidates with a background in home health physical therapy or experience in a similar role will be highly regarded. A creative approach to problem-solving and a strong passion for improving patient healthcare pathways are also desirable traits.

We offer a competitive salary range of USD 100,000 - 120,000 annually, complemented by a comprehensive benefits package tailored for full-time team members.
