START Theraputic In-Home Coach

Serves as the primary contact supporting individuals in the coaching program with Developmental Disabilities and Mental Health challenges for the START Program (Systematic, Therapeutic, Assessment, Resources, and Treatment) to provide community based intervention and prevention services to active participants within START. The program relies on integrated care and treatment with collaboration among all team members. The services in the START Program include collaborative contacts, after hour contacts, emergency team meetings, therapeutic coaching and therapeutic respite.

Position Requirements:

Education:

  • Bachelor's degree in psychology, Social Work or otherHuman Servicerelated field required with 2 years' experience with population servedpreferred

Populations Served Include:Intellectual Developmental Disability,Mental Health (Child and Adult)

EssentialJob-RelatedResponsibilities

  • Coachesworkwithinthehome or othercommunitysettings to create andimplement person specific and crisis therapeutic interventions and communicate these interventions with primary caregivers.

  • Coaches contribute to the overall crisis stabilization and prevention of the individual and family byprovidinghands-on assistance and education to families.

  • Coach participates as partofcomprehensive START services that collaborate with individual's primary provider and clinical home, communitycrisisservices,familyandothers.

  • Offer consultation and hands-on education withentirefamily, support system to create a safe, nurturing, and meaningful environment for families and individuals with development disabilities.

  • Assistsindividualswith implementationofdaily activities, behavioral health needs, and health, wellness, and leisure activities.

  • Monitors individualsforanymentalhealthorbehavioralsymptomsand providescrisisprevention, interventionandde-escalation support inaccordancewithcrisisguidelinesandperson'splan.

  • Maintains timely documentation of session and visits notes and other data.

  • Must be self-motivated, efficient with time

  • Coaches work with families and individuals to work on identified needs/skills to assist with

  • strategies to improve coping mechanisms

  • Coaches help to develop strategies and tools to assist others with improving overall well-being.

  • Name checked in the registries. (OIG exclusions database, Child Abuse Registry, and Offenders Against Individuals with Developmental Disabilities)
  • Drug testing
  • Education verification and other credentialing based on position requirements.
  • Proof of employment history or references (if required)
  • Positions that require driving Proof of driver's license, driver's insurance, and vehicle, IF required for providing transportation for individuals.
