(FDB) is currently seeking an Advanced Integration Engineer (Epic) who is responsible for designing, developing, deploying, and supporting secure, scalable healthcare interoperability solutions across major EHR platforms, including Epic, Meditech, and Oracle Cerner. This role owns end-to-end interface delivery and production reliability for HL7 v2 messaging, FHIR R4 APIs/services, SMART on FHIR workflows, and EHR application registration.
The position partners with client IT teams, EHR analysts, and internal engineering/product teams to translate clinical and operational needs into robust technical integrations that meet quality, compliance, and performance standards.
The position will be based out of the Carmel, IN office and follows a hybrid work model, with an expectation to work onsite at least three days per week.
Responsibilities:
  • Lead design, development, and lifecycle management of integrations with Epic, Meditech, and Oracle Cerner.
  • Build and maintain interfaces using HL7 v2 standards (including ADT, ORM, ORU, SIU, and MDM) and related healthcare message workflows.
  • Design, implement, and support FHIR R4-based APIs and services for clinical and administrative data exchange.
  • Implement and support SMART on FHIR launch and OAuth2 authorization flows, including scopes and token lifecycle management.
  • Support Epic application registration workflows (including App Orchard processes, OAuth client setup, and environment onboarding).
  • Develop and support modern web services and APIs using technologies such as C#, JavaScript/TypeScript, .NET, and related frameworks.
  • Follow strong engineering practices using Git and GitHub workflows, including pull requests, branching strategies, and peer code reviews.
  • Own integration workstreams end-to-end: requirements gathering, technical design, development, testing, deployment, and production support.
  • Partner with client implementation teams to define interface specifications, data mappings, validation criteria, and go-live readiness plans.
  • Develop transformation logic, error-handling patterns, and resiliency controls across integration pipelines.
  • Create and maintain integration documentation, including interface control documents, runbooks, support guides, and architecture diagrams.
  • Implement proactive monitoring, alerting, and observability for interfaces and FHIR services; drive rapid root-cause analysis and incident resolution.
  • Ensure integration solutions align with security and compliance requirements, including HIPAA, least-privilege access, and secure data handling practices.
  • Collaborate across engineering, product, and operations to prioritize integration roadmaps and resolve cross-functional dependencies.
  • Provide technical leadership across integration initiatives, including architecture guidance, delivery planning, and risk management.
  • Mentor peers on interoperability standards, implementation approaches, and engineering best practices.
  • Lead technical design and code review sessions; establish and reinforce development standards for integration and API services.
  • Apply AI-assisted engineering practices using coding agents (such as Claude Code, Cursor, and similar tools) to improve delivery speed and code quality.
  • Drive spec-driven AI development by translating requirements into clear technical specifications, acceptance criteria, and implementation plans that can be executed reliably by AI-enabled workflows.

Required Experience:
  • Bachelor's degree in Computer Science, Information Systems, Engineering, or related field preferred.
  • 5+ years of healthcare integration and/or software engineering experience with direct interoperability delivery ownership.
  • Demonstrated expertise in HL7 v2 messaging and healthcare interface implementation across multiple trading partners.
  • Hands-on experience building and supporting FHIR R4 integrations and services in production environments.
  • Practical experience with SMART on FHIR application integration, OAuth2/OpenID Connect concepts, and API security controls.
  • Proven experience with Epic, Meditech, and Oracle Cerner integration patterns, tooling, and deployment practices.
  • Strong background in RESTful API development, JSON/XML data modeling, and message transformation techniques.
  • Experience with one or more integration platforms/middleware tools (Mirth Connect, Rhapsody, Cloverleaf, HealthShare, MuleSoft, or equivalent).
  • Proficiency in one or more development languages such as C#, JavaScript/TypeScript, Python, or Java.
  • Experience designing and maintaining SOAP/REST web services in enterprise healthcare environments.
  • Strong experience with Git/GitHub source control workflows, CI/CD fundamentals, and production support practices.
  • Practical experience using AI coding agents (for example, Claude Code, Cursor, or similar tools) in day-to-day engineering workflows.
  • Experience with spec-driven development approaches, including creating implementation-ready technical specs and testable acceptance criteria.
  • Strong analytical, troubleshooting, and communication skills with both technical and non-technical stakeholders.
