For over 125 years, Calvary Hospital and Calvary @Home located in the Bronx, New York has been dedicated to the palliative care of advanced cancer and other terminally ill patients and their families. Through a caring, compassionate, philosophy of non-abandonment, the hospice provides a full spectrum of palliative services, including emotional, spiritual, and physical care. The hospice receives patients from referring institutions throughout New York City and the entire tristate area. Requirements
The Hospice Intake Nurse coordinates Agency intake, participates in the planning, implementation, documentation and coordination of patient care services to ensure that services are consistent with the philosophy, policies and standards of the Agency.
  • Coordinates intake for the Calvary Hospital Hospice/Home Care.
  • Assists in the explanation of the Agency's eligibility criteria and reimbursement sources to patients, families and community. May attend Discharge Planning Conferences at local hospitals to plan for Hospice/ Home Care Services.
  • Serves as educational liaison to hospital(s) concerning the range of Hospice and Home Health Services. Communicates, interprets and promotes Agencies' philosophies, policies and procedures to discharge planning, nursing, medical and other hospital staff, as well as community agencies, patients and families.
  • Assists in the development of policies and procedures for effective intake of cases and communication of intake information.
  • Collects third party payor information and communicates with billing office to ensure acquisition of correct billing information.
  • Communicates regularly with management of Hospice and Home Care to assure a smooth flow of information.

Qualifications:
  • Current New York State Registered Nurse License/Permit.

Education:
  • Associate Degree
  • BSN from an accredited school is preferred.

Salary: $113,576 - $140,076 annually
