Role Overview:

Grace Healthcare is seeking a highly skilled and detail-oriented Nurse Informatics Support RN with strong clinical knowledge and direct experience using Home Care Home Base (HCHB). This role serves as a bridge between clinical teams and IT, ensuring that our documentation systems and workflows support safe, efficient, and compliant patient care delivery. The Nurse Informatics Support RN will train and support staff, optimize system use, and assist with troubleshooting and quality improvement efforts related to electronic clinical documentation.

Key Responsibilities:

  • Serve as the HCHB subject matter expert (SME) for clinical documentation workflows
  • Provide ongoing training and support for field clinicians (RNs, LPNs, HHAs, Social Workers, etc.)
  • Conduct system audits to monitor documentation compliance and accuracy
  • Partner with clinical leadership to identify workflow gaps and implement solutions
  • Collaborate with IT to escalate system issues, track resolutions, and implement updates
  • Participate in new employee onboarding and system orientation
  • Assist with the design and roll-out of clinical templates, forms, and care plans in HCHB
  • Support regulatory compliance efforts related to EMR documentation (Medicare, CHAP, etc.)
  • Provide one-on-one coaching to staff struggling with electronic documentation
  • Track and report system utilization metrics to leadership
  • Stay current on industry best practices in home health informatics and HCHB updates

Qualifications:

  • Registered Nurse (RN) licensed in the state of [Insert State]
  • Minimum of 2 years clinical nursing experience, preferably in home health or hospice
  • Minimum of 1 year direct experience with Home Care Home Base (HCHB)
  • Strong understanding of clinical documentation standards and compliance requirements
