Case Manager/Service Coordinator for dual population. 2 programs, mixed caseload. (aging population that wish to live in their home, NOT in a nursing home) AND TBI survivors. Salary commensurate with experience. Bachelors AND 3+ years experience required

Looking for a Case Management Supervisor to work a part time caseload (see Service Coordination) job duties below.

The supervisor is expected to meet any waiver participants prior to the completion of the Revised Service Plan (RSP) developed by a Service Coordinator under their supervision. Any visits that exceed 10 in a month will be assisted by the TLC Directors. The Supervisor will have supervisory meetings with staff on at least a monthly basis, and review and sign-off on all service plans and monthly notes. In addition, they will monitor the quarterly tracking spreadsheet to ensure proper time management by staff.

A supervisor MAY maintain an active caseload of waiver participants up to 25. However, this caseload must be reduced from
the maximum limit allowed in relation to his/her supervisory responsibilities.
The Service Coordinator Supervisor must complete a monthly face to face visit with each TBI waiver participant on their caseload. They must meet with the participant in their home at least quarterly.

Job duties

The Service Coordinator will also be responsible for:

  1. Formally reviewing, updating and submitting timely Service Plans (ISP, RSP, and addendum) to the RRDS for review (refer to the Service Plan section of this Manual for specific information on Service Plan responsibilities);
  2. Organizing and facilitating Team Meetings; 30 day meeting and 6 month meeting
  3. Maintaining records for at least seven years after termination of waiver services;
  4. Assuring that the UAS is completed annually and meets LOC (Level of Care):
  5. Maintaining records of waiver transportation as described in the Manual section on Transportation Services;
  6. Assuring that all waiver service providers and the participant receive a current copy of the most recently approved Service Plan;
  7. Maintaining knowledge of all approved waiver service providers in their region;
  8. Conducting in-home visits with the participant no less than once a quarter, virtual visits monthly; and
  9. Create and Review Housing documents for annual housing audit for participants that use housing subsidy

Qualifications needed

Persons employed as Service Coordinators must be a/an:

1. Licensed Master Social Worker (LMSW- licensed by the NYS Education Department);

2. Licensed Clinical Social Worker (LCSW- licensed by the NYS Education Department);

3. Individual with a Doctorate or Master of Social Work degree;

4. Individual with a Doctorate or Master of Psychology degree;

5. Licensed Physical Therapist (licensed by the NYS Education Department);

6. Registered Professional Nurse (licensed by the NYS Education Department);

7. Certified Teacher of Students with Disabilities (certified by the NYS Education Department);

8. Certified Rehabilitation Counselor (certified as a Certified Rehabilitation Counselor (CRC) by the Commission on Rehabilitation Counselor Certification);

9. Licensed Speech Pathologist (licensed by the NYS Education Department);

10. Licensed Occupational Therapist (licensed by the NYS Education Department);

AND

Must have, at a minimum, one (1) year of experience providing case management/service coordination, information and referral services and service brokering of community based services for individuals with disabilities and/or Traumatic Brain Injury;

OR

Individual with a Bachelor’s degree and three (3) years of experience providing case management/service coordination, information and referral services and service brokering of community based services for individuals with disabilities and/or Traumatic Brain Injury.
