REGISTERED NURSE (RN)- Primary Care
Are you passionate about advancing health equity and delivering patient-centered care to underserved communities?
Western North Carolina Community Health Services (WNCCHS) provides patient-centered, team-based primary care to residents of Western North Carolina. We are looking for a full-time (40 hours per week) RN to join our growing team!
The Clinic Nurse provides coverage for our
McDowell Health Center, 136 Creekview Ct, Marion, NC 28752
RESPONSIBILITIES:
Patient Care
The clinic nurse delivers comprehensive, patient-centered care, in collaboration with the healthcare team, focusing on health promotion, disease prevention, and chronic illness management. Key responsibilities include patient assessments, administering vaccinations and routine medications, managing chronic conditions, performing screenings, and patient education. The clinic nurse assists with walk-in and phone triage.
Key Responsibilities and Duties
  1. Clinical Assessments & Procedures: Conducting physical exams, measuring vital signs, wound care, ear syringing, and administering immunizations/injections, completing co-visits with medical providers
  2. Chronic Disease Management: Monitoring and managing conditions such as diabetes, asthma, and hypertension, including medication adjustments based on protocols, and counseling.
  3. Patient Education & Counseling: Providing guidance on healthy lifestyles, managing illnesses, and preventive health measures.
  4. Administrative & Coordination: Documenting medical histories, managing patient records, and coordinating care with other specialists.
  5. Triage: Providing over-the-phone and in person triage to assess patient needs

Miscellaneous
  1. Participates and/or leads quality improvement activities
  2. Maintain current knowledge of clinical best practices, guidelines, and evidence-based standards, and integrate them into daily nursing practice.

Qualifications
  • Licensure: Current license to practice as a Registered Nurse in North Carolina
  • Degree: Bachelor of Science in Nursing (BSN) or associate's degree in nursing (ADN)
  • Skills: Strong communication, clinical assessment skills, proficiency in electronic medical records (EMR), and the ability to work in a team-based environment.
  • Certification: BLS /CPR Certified

Preferred Qualifications:
  • Proficient in Microsoft Office programs
  • Minimum of two (2) years' experience in an outpatient setting.
  • Familiarity with role of FQHC in the community.
  • Bilingual in Spanish preferred
  • Women's Health/Prenatal experience
