Position Highlights
Monday-Friday | 8:00 AM-5:00 PM
Shared call rotation
In-person and virtual visits supported by Medical Assistant
Manage complex patients with ongoing follow-up throughout stay
Collaborate with SNF team to facilitate care coordination
Implement discharge planning to improve patient outcomes
Signing bonus, CME, and relocation available.
