Job description

POSITION: Clinic Registered Nurse
DEPARTMENT: Clinic
HOURS: 72 hours per pay period/Non-Exempt
HOURLY WAGE: $36.27 - $46.63
GENERAL RESPONSIBILITIES:
Provides a team approach in coordination with providers and support staff in the provision of care in the clinic setting. Effective communication with provider/nurse teams. The Registered Nurse (RN) will provide a variety of tasks including but not limited to, annual wellness visits, care coordination, assistance with treatments and procedures, education, injections, triage, medication refills, and assisting the Physicians/Advanced Practice Providers with patient care as needed.
Maintain a high standard of quality healthcare. Practice within the scope of their licensure.
Works in collaboration and continuous partnership with patients including chronically ill or "high risk" patients and their family/caregiver(s), clinic/hospital/specialty providers and staff and community resources in a team approach to:
  • Promote timely access to appropriate care
  • Increase utilization of preventative care
  • Reduce emergency room utilization and hospital readmissions
  • Promote patient education
  • Develop and promote adherence to a care plan in coordination with the patient, primary care provider, and family/caregiver(s)
  • Increase continuity of care by managing referrals
  • Increase patients' ability for self-management and shared decision-making

EDUCATION/TRAINING:
Graduate of an accredited school of registered nursing. Must have and maintain a current unrestricted Minnesota Registered Nursing license and CPR certification.
