• Conduct thorough virtual psychosocial assessments to understand clients’ strengths, challenges, and unique needs in a hospice setting.
• Design and implement evidence-based treatment plans that address the emotional, social, and psychological aspects of clients and their families.
• Facilitate individual and group counseling sessions, utilizing various therapeutic approaches such as Cognitive Behavioral Therapy (CBT) and Acceptance and Commitment Therapy (ACT).
• Work closely with medical professionals including physicians, nurses, and nutritionists to create a comprehensive care strategy that supports the best possible client outcomes.
• Advocate for clients’ access to community resources such as support groups or financial assistance, ensuring they receive the holistic care they deserve.
• Maintain detailed and accurate records of client interactions, treatment progress, and therapeutic outcomes in our electronic health record system, ensuring adherence to confidentiality and compliance standards.
• Actively participate in case conferences and team meetings, contributing insights and learning from peers to continuously improve clinical practices.

Your primary focus will be on clients in hospice care, a sensitive area that requires not only clinical expertise but considerable empathy. You will support individuals facing terminal conditions and their families, helping them navigate grief, anxiety, and other emotional challenges associated with end-of-life issues. Your role will empower clients to articulate their needs and foster deeper connections within their families during this challenging time.

• A Master's degree in Social Work from an accredited program.
• Current licensure as a Licensed Clinical Social Worker (LCSW) in California.
• A minimum of two years of post-licensure experience in clinical social work, particularly in hospice settings preferred.
• Proficient in performing complex diagnostic assessments and developing targeted treatment plans.
• Exceptional interpersonal skills with the ability to build rapport and trust with clients from diverse backgrounds.
• A strong commitment to maintaining ethical standards, cultural competence, and advocating for social justice.

• Previous experience working in a hospice or palliative care setting is highly desirable, as it will inform the tailored approaches necessary for this unique patient population.
• Familiarity with telehealth technologies and remote counseling practices.
• Certification in evidence-based practices, such as DBT or ACT, that align with our therapeutic modalities.
• Experience with electronic health record (EHR) systems for efficient documentation and coordination.

The position offers a competitive salary ranging from USD 115,000 to USD 130,000 annually, commensurate with your experience and caseload. In addition to a generous salary, we provide:
