• Conduct comprehensive virtual psychosocial assessments to identify clients' strengths, needs, and challenges, utilizing best practices to guide your evaluations.
• Provide consistent evidence-based therapeutic interventions, including individual and group therapy sessions tailored to meet the unique needs of clients and families.
• Collaborate seamlessly with a multidisciplinary team that includes healthcare providers, hospice staff, and other social workers to create integrated care plans that prioritize clients’ overall wellness.
• Advocate for client rights and access to critical community resources, ensuring they receive the necessary support services during their care journey.
• Maintain meticulous electronic health records documenting client interactions, treatment progress, and outcomes to comply with relevant regulations and promote high standards of care.
• Engage in continuous professional development by participating in team meetings, case conferences, and further educational opportunities designed to enhance your clinical skills.

Your work will primarily focus on supporting patients in a hospice setting, engaging with individuals experiencing complex health considerations and end-of-life challenges. This role requires sensitivity, empathy, and the ability to build strong rapport with clients from diverse backgrounds, empowering them to express their feelings and preferences in a safe environment.

• A Master’s degree in Social Work from an accredited institution.
• Active licensure as a Licensed Clinical Social Worker (LCSW) in the state of California.
• A minimum of two years of post-licensure experience delivering clinical social work services, with a specific focus on mental health treatment.
• Demonstrated proficiency in diagnostic assessments and treatment plan development, showcasing the ability to tailor approaches based on individual client needs.
• Strong interpersonal and communication skills, with a commitment to fostering rapport with a variety of populations.
• Dedication to cultural competence, ethical practice, and social justice, consistently promoting an inclusive environment for clients.

• Experience working in a hospice, palliative care, or similar settings, providing comfort and support to patients and families during difficult times.
• Certification(s) in evidence-based therapies such as Dialectical Behavior Therapy (DBT), Acceptance and Commitment Therapy (ACT), or other relevant modalities.
• Established track record of advocacy and resource connection for clients facing socio-economic challenges.
