Position Summary

As a Hospice CNA, you will work closely with our interdisciplinary team to provide high-quality personal care that promotes dignity, comfort, and quality of life for patients receiving hospice services in patients' homes and care facilities.

Responsibilities
  • Provide personal care including bathing, grooming, dressing, toileting, and mobility assistance.

  • Observe and report changes in patient condition to the supervising nurse.

  • Document patient care accurately and timely.

  • Promote patient comfort, dignity, and independence.

  • Build compassionate relationships with patients and their families.

  • Follow all hospice policies, procedures, and infection control standards.

Qualifications
  • Current Pennsylvania Certified Nursing Assistant (CNA) certification.

  • Valid driver's license and reliable transportation.

  • Ability to travel throughout the assigned service area.

  • Excellent communication and interpersonal skills.

  • Hospice, home health, or long-term care experience is preferred but not required.
