Vista Pine Haven, are well-established and trusted providers in the senior living space located in Pine Island, MN, and a proud member of Vista Senior Living Management, is looking for a compassionate and driven Director of Wellness who will lead our clinical team and help create an exceptional experience for our residents and caregivers alike.

Position: Director of Wellness

The ideal candidate will be a high-energy, self-driven leader who empowers and motivates those around them. In this role, you will be responsible for managing the day-to-day clinical services of Vista Pine Haven, this includes providing the supervision necessary to lead, coach, and mentor direct care staff within the community.

Responsibilities:

  • Oversees day-to-day clinical services with the support of an Assistant Director of Wellness (ADOW) and Resident Care Coordinator (RCC) to ensure residents’ healthcare needs are met
  • Provides leadership, coaching, and guidance to direct care staff, fostering a collaborative and supportive clinical environment
  • Works alongside the ADOW and RCC to lead the clinical leadership team, including Resident Care and Staffing Coordinators
  • Empowers clinical leaders to supervise, direct, and motivate staff within their areas of responsibility
  • Partners with clinical leaders to assign and prioritize work, support performance management, and address concerns as needed
  • Supports training and education for caregivers during orientation and through ongoing classroom in-services and situation-specific training
  • Assists with planning work schedules, assigning duties, and providing day-to-day direction to ensure smooth clinical operations
  • Reviews and monitors clinical documentation completed by caregiving staff to ensure accuracy and compliance
  • Participates in pre-admission screening of prospective residents in collaboration with the leadership team
  • Conducts or supports on-site evaluations for residents admitted to alternate care settings and maintains communication with resident families to support transitions back to the community
  • Ensures required documentation is completed timely for admissions, including nursing assessments, service plans, and related forms
  • Monitors changes in resident condition and collaborates with the Executive Director, physicians, and families as appropriate
  • Facilitates continuity of care for residents receiving home health, hospice, and other third-party healthcare services
  • Participates in or leads resident care–related meetings, such as Service Plan meetings, with appropriate team members
  • Ensures incident reports are completed accurately and in a timely manner
  • Participates in campus Quality Improvement initiatives to support high-quality resident care

Qualifications:

  • Registered Nurse (RN) in the state of Minnesota
  • Two (2) years of experience in a nursing position, preferably from a Long-Term Care, Assisted Living, and/or Memory Care community
  • Supervisory and leadership skills with the ability to effectively supervise staff
  • Excellent communication and human relations skills
  • Ability to maintain and protect the confidentiality of information
  • Knowledge and/or experience in the areas of geriatric nursing or can learn quickly
  • Caring and compassionate disposition
  • CPR Certified
