Description

American Traveler is seeking an experienced RN for a home-based Hospice and Palliative Care position in New Jersey, requiring a minimum of 2 years of home hospice RN experience and a valid NJ driver's license.

  • Home-based hospice and palliative care setting serving patients in their place of residence
  • Day shift schedule, 8amโ€“4pm, 5 days per week (37.5 hrs/week)
  • 16-week contract
  • One weekend of on-call coverage required per month
  • Floating to different counties required based on patient need
  • EMR: Epic (experience required)
Requirements
  • Active NJ RN license (NJ or Compact) required
  • Current BLS certification required
  • Minimum 2 years of RN experience in a home hospice environment required
  • Valid New Jersey driver's license required
  • Associate's degree (ASN) or RN Diploma required
  • Proficiency with EMR systems required; Epic experience preferred
  • Strong critical thinking and communication skills required
  • 2 professional references required for consideration
Additional Information
  • Conducts patient assessments to evaluate appropriateness for hospice care and develops individualized plans of care for patients and families
  • Manages a caseload as primary case manager, including developing, implementing, and evaluating evidence-based care plans
  • Performs laptop data transfers twice daily and completes admission assessments and documentation within required timeframes
  • Attends interdisciplinary team (IDT) meetings every two weeks and collaborates with the full care team to communicate patient progress
  • Responsible for medication reconciliation, safe medication management, and maintaining current medication orders
  • Provides patient and family education using age-appropriate teaching methods and documents all education tools used
  • Participates in performance improvement activities, UR meetings, and provides mentorship and orientation to new staff as requested
  • Candidates must reside more than 50 miles from the facility โ€” local candidates are not eligible for this position
  • Travelers may float to a nearby sister facility as needed
