Job Summary
Our client is seeking a dedicated Case Manager who will be responsible for managing client cases by conducting screenings and assessments, developing service plans, and coordinating resources. The ideal candidate will have strong client engagement skills and the ability to connect clients with necessary community resources and services.
Responsibilities / Job Duties
  • Screen clients for HIV/AIDS case management eligibility and complete intake processes.
  • Conduct client assessments to identify needs, barriers, and priorities, and evaluate psychosocial support needs.
  • Collaborate with clients to build comprehensive service plans and align services with identified needs.
  • Connect clients with community resources and coordinate referrals.
  • Maintain monthly client contact and track progress against service plans.
  • Advocate for clients to access appropriate services and support problem-solving related to care needs.
  • Participate in food and transportation service delivery and reconcile data against service delivery.
  • Facilitate or organize support groups, educational seminars, and participate in community events.
  • Enter services into the database timely, prepare service reports, and support quality improvement activities.

Minimum Education and Experience Qualification Requirements
Qualifications
  • 1-3 Years of relevant experience.
  • Experience working with diverse cultural and socioeconomic backgrounds.
  • Comfortable with client-facing work, documentation, and follow-up.

Skills
  • Proficiency with computers and standard office hardware.
  • Familiarity with Microsoft applications and case management software.

Schedule / Shift
Schedule: 8:45 AM to 5:30 PM Monday to Friday
