We are a private home care provider that assist people with chronic medical and physical conditions with the necessary resources and personal care assistance they need while living in their homes. MCMS uses the trans-disciplinary team approach to serve those who are in need of Nursing Services, Personal Care, and Companion/Sitter care.
Job Description
The Home Care Healthcare Consultant/Director is primarily responsible for facilitating the communication and transition of care between referral sources, patients, and families to home care to increase and/or maintain high levels of participation. Incumbent is responsible for working directly with physician/physician groups and their staff, hospital administrators and staff, patients/families, discharge coordinators, case management staff to educate, report trends and progress, promote the services of the Agency by coordinating the business activities, transitions of care and admission process. Plan, develop, and coordinate home care referrals for patients. Serves as an educational resource to healthcare facilities and the community regarding home care services. Plans, implements and conducts educational home care programs and presentations.
Primary duties are to develop and maintain new accounts and markets to the community as directed by the Director of Business Development and/or Administrator. Lend customer service support to designated referral contacts and maintain current account information.
Qualifications
At least 2 years experience in community resources. Knowledge in medical professional community. Ability to connect and collaborate with families and community.
Additional Information
Salary is based on experience and will be addressed upon interview.
This position is a Part-time/PRN position and requires some weekends and on-call.
