Job description

NATURE OF WORK: Provide personal care and habilitation services to individuals with intellectual/developmental disabilities primarily in individualized support settings (ISS). The HCA will assist individuals with I/DD in their Activities of Daily Living, as well as implement individualized habilitation programs of training and active treatment as established by the consumers treatment team. These services are designed to provide skill development to individuals with I/DD and 24-hour protective oversight and supervision.
MINIMUM QUALIFICATIONS:
  • Ability to perform Essential Duties as outlined below.
  • High School Graduate or equivalent.
  • Valid Drivers License
  • Ability to obtain and maintain CPR, First Aid, Mandt certifications and other required trainings
  • Ability to comply with Clients Rights
  • Ability to comply with Valleys and division departmental safety procedures.
  • Ability to read, write and speak the English language
  • Ability to use E-Mail and document services in an Electronic Medical Record (EMR)

PREFERRED QUALIFICATIONS:
  • Previous experience working with I/DD population or behavioral health background.

WORK ENVIRONMENT: In consumers home location 85% of the time; outdoors and transportation areas 15% of the time.
WORK PLACE: Pace determined by the need of client(s).
ESSENTIAL DUTIES:
  • Accountable for personal needs (i.e. grooming, toileting, changing, dressing, nutrition). Employee will ensure that personal care, change in clients health status and hygiene of all clients is maintained as client needs dictate.
  • Complete documentation as required by Valley, Medicaid, OSHA, and other licensing agencies. Employee will ensure that documentation (such as Direct Services Progress Notes, Task Analysis Forms, Behavior Tracking Forms, DD Forms, etc.) are completed accurately, appropriately, and timely in the Electronic Medical Record.
  • Perform all-predetermined services as per doctors orders. Assure that the safety, health and well-being of clients are always maintained on or off site.
  • Implement treatment/behavior plans in compliance with Valleys and State guidelines.
  • Employee will engage clients in achieving their daily goals and objectives through active treatment.
  • Employee will complete all job duties as assigned within the specified timeframe or shift as scheduled. In doing such duties, all Valley Policies and Procedures as well as State and Federal regulations will be complied with in a promptly and timely manner. Examples of such standard duties include but are not limited to completing shower schedules, dressing and changing clients appropriately, preparing and following meals plans and schedules, notifying the supervisor of items needed by the clients, completing shift to shift reports, following cleaning schedules, accompanying clients to doctor appointments as schedule, completing van runs, reporting incidents to supervisors, completing van logs and following van procedures, completing work orders, etc.
  • Monitor, maintain and use appropriately, all equipment and supplies.
  • Transportation of clients to appointments/activities in the community using only a company vehicle.
  • Support and help in the training of new employees.
  • Attends all mandated trainings related to profession and enhances knowledge and/or skills, which includes Essential Learning Trainings.
  • OTHER DUTIES AS ASSIGNED.
